On January 28, 2025, we discovered that an unauthorized person gained access to our computer system. It is possible that the unauthorized person accessed the personal or health information we have on file for you, including, for example, your SSN, account information, financial information, medical records, or disability-payment information.

We have retained a forensic firm, who have begun an investigation. We are also working to provide two free years of credit monitoring for you, and we will send you details shortly. Meanwhile, we encourage you to be vigilant by reviewing your account statements and credit reports for any unauthorized activity.
